“To improve diagnostics of the upper gastrointestinal tract disorders in children with chronic dyspepsia using а novel respiratory test”

Some peculiarities in various nosologic forms of gastroduodenal pathology (functional dyspepsia, chronic gastroduodenitis, and ulcerous disease) have been established in adolescents in the course of carrying-out the research project .
Research object: clinical, morphological and functional manifestations of gastroduodenal pathology (GP) in 130 adolescents aged 10-18 years who were under observation in the in- patient clinic of the Institute.
Objective: to improve certain methods for an early diagnosis of the digestive system diseases in adolescents, based on the study of the exhaled gas parameters using sensory analysis.
Methods: conventional clinical, questioning, instrumental (FGDS, pH-metry, dynamic gastroscintigraphy), HRP, Helic- test, immunofermental, and respiratory test (sensory analysis).
Theoretical and practical results: the incidence of the risk factors for functional dyspepsia, chronic gastroduodenitis and peptic ulcer disease development in adolescents has been established, and some mechanisms of said pathologies (the role of H. pylori, peptic factor, motor and evacuatory disorders) in the formation of functional disorders, inflammation and destruction have been verified. Prognostic tables for chronic gastroduodenal diseases (ChGD) development in adolescents with dyspeptic complaints have been singled out. Diagnostic markers of the exhaled gas in adolescents with gastroduodenal pathology (GP) have been presented. An algorithm for an early ChGD detection in adolescents with dyspeptic complaints with the use of prognostic tables and sensory analysis of the exhaled gas has been proposed.
Novelty: some peculiarities in manifestation of various GP nosological forms in adolescents with dyspeptic complaints as well as certain mechanisms of their formation have been established. It has been shown that the clinical course of functional, inflammatory and destructive gastroduodenal diseases (GD) is characterized by a considerable diversity of complaints and a significant delay of gastric motility. The greatest incidence and severity of gastric dysrhythmical patterns is observed in adolescents with functional dyspepsia, which makes it possible to consider them as the main factors of the disease, whereas in ChGP the principal part belongs to H.pylori infection and peptic factor, realized against the background of burdened heredity as regards GD, including social risk factors.
Effectiveness: the work solves an important problem of pediatrics and gastroenterology concerning improving a technology for early diagnosis of the upper gastrointestinal tract diseases in adolescents, basing on the results of the latest research methods and taking into account the parameters of the exhaled gas.
Application fields: out-patient clinics, departments of pediatrics and gastroenterology at the in-patient clinics of the district, town, regional and national hospitals, and medical institutions of higher education.
